    • Marriage certificate.
    • Court order.
    • Divorce decree.
  • Proof of your identity, such as your:
    • NM driver's license or ID card.
    • Military ID.
    • Passport.
  • Proof of your U.S. citizenship (if required) OR lawful presence in the U.S., such as your:
    • U.S. birth certificate.
    • Work permit and I-94.
    • U.S. passport.
  • Application for a Social Security Card (Form SS-5).
