Odometer Disclosure Statement

When transferring a vehicle title and changing ownership, owners must submit an accurate odometer reading. To do this, many state motor vehicle agencies provide a form called the Odometer Disclosure Statement. In some cases, you can simply fill in the odometer reading section on the certificate of title or registration application. Find your state's form below:
